POSITION SUMMARY
The Case Management Social Worker performs effective assessments of patients and their families.
Based on the needs identified in an assessment, the Social Worker arranges, coordinates, and monitors multiple services from different providers to meet patient’s needs.
The Social Worker addresses the psychological and social needs of the patients and formulates the discharge plan in collaboration with all appropriate team members.
Performs needs assessments; prioritizes patients' needs / choices; and collaborates with the interdisciplinary team; patient / pertinent other; and community resources in order to fully develop/organize treatment plans with optimal outcomes.
Coordinates plans with patient / family; members of the health care team utilizing best practice approaches.
Performs on-going evaluation / readjustment of plans for efficacy; quality; cost-effectiveness; and goals / outcomes.
Serves as patient liaison / advocate between care providers; community agencies; and insurance payers.
Advocates for services and funding necessary to meet established outcomes.
Maintains working knowledge of payer requirements.
Prepares and maintains required documentation for each case.
Complies with State and Federal mandates for documentation and reporting.
Responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health
Assumes responsibility to assess the patient for transition needs including identifying and assessing patients at risk for readmission.
Under the guidance of the Case Management Supervisor, the Social Worker is responsible for conducting and coordinating patient assessments, transition of care, discharge planning affecting patient outcomes, and patient/family education.
Our ideal candidate will be a self-starter who is able to work independently and has the following mandatory skills and experience:
Demonstrated evidence of dependability, diplomacy, and initiative
Demonstrated strong communication and interpersonal skills with all levels of internal and external customers, including but not limited to medical staff, patients and families, clinical personnel, other Case Managers and Social Workers, support and technical staff, outside agencies, third party payors, and members of the community
Basic understanding of reimbursement issues, denials, and DRGs
Personal computer skills using e-mail, Microsoft Office Suite
Ability to learn how to use the electronic medical record and other computer software as needed.
Ability to understand health concepts and terminology
Licenses and Certifications Required
Licensed Social Worker in the state of North Dakota
BLS certification required within orientation period
Educational Requirements
Bachelor’s degree in social Worker Required
Preferred Requirements
Two years of experience in the area of social work preferred, particularly in an acute care setting
